NHTD / TBI
At Welcome Care Inc, aging shouldn’t mean giving up your independence. Through the Nursing Home Transition and Diversion (NHTD) Program, we help Medicaid-eligible individuals remain safely in their homes and communities—rather than moving to a nursing facility.

NHTD Program
Our Nursing Home Transition & Diversion (NHTD) program is designed to help you live with independence, dignity, and comfort in your own home and community. We understand that many people prefer to avoid moving into a nursing facility, and this program provides the right support to make that possible.
The NHTD (Nursing Home Transition & Diversion Waiver) is a New York State Medicaid program that allows individuals who qualify for nursing home–level care to receive those same services at home. It supports both people currently living in institutions who wish to return to the community, and those who want to remain at home rather than transition to a facility.
