NHTD Program

Our Nursing Home Transition & Diversion (NHTD) program is designed to help you live with independence, dignity, and comfort in your own home and community. We understand that many people prefer to avoid moving into a nursing facility, and this program provides the right support to make that possible.

The NHTD (Nursing Home Transition & Diversion Waiver) is a New York State Medicaid program that allows individuals who qualify for nursing home–level care to receive those same services at home. It supports both people currently living in institutions who wish to return to the community, and those who want to remain at home rather than transition to a facility.

The NHTD Program is open to New York State residents who fulfill the eligibility requirements:

Must Have Medicaid Eligibility

To enroll in the NHTD program, individuals must qualify for community-based Medicaid.

Can Live Safely at Home with Support

Applicants must be able to remain safely in their home or community when supported by services offered through the NHTD program, Medicaid, and other available resources.

Adults with Disabilities or Seniors

Eligible individuals include adults ages 18–64 with a documented physical disability, or anyone 65 and older at the time of application.

TBI Program

Our TBI Program is designed to support individuals living with the long-term effects of a traumatic brain injury or similar non-degenerative conditions. Through personalized care planning, rehabilitation support, and community-based services, we help participants regain independence, build confidence, and live safely at home.

We work closely with individuals, families, and healthcare providers to create care plans that reflect each person’s goals, strengths, and challenges—ensuring dignity, safety, and quality of life every step of the way.

The TBI program is open to New York State residents who fulfill the following eligibility requirements.

Live in Community, Not in a Facility.

The TBI program supports individuals who prefer to remain in their communities instead of moving into a nursing home, while still receiving the care they need.

Be assessed as needing nursing care to live safely in the community.

Needing a level of care typically provided in a nursing facility, but with proper support and a safe environment, you can live successfully in the community.

Aged 18 to 64 with TBI or Similar Condition.

Applicants must have a primary diagnosis of TBI—or a non-degenerative condition with similar cognitive or physical impairments, such as stroke—that significantly impacts daily functioning.
